To change your name after marriage, fill the application form in the Gazette of India. Along with the application, submit your marriage certificate, affidavit, copies of proof of marriage, your old and new name, husband/father’s Aadhaar card and other necessary documents in a file to the Gazette office. After the file is verified, the name change is published in the Gazette.
