To change one's name, an affidavit is first prepared, which includes the following details:

• Old Name
• New Name
• Reason for the Change
• Full Address

This is printed on non-judicial stamp paper and notarized.

Newspaper Name Change Ads

• Get your name change notice published in a newspaper.
• The newspaper must be local to the state.
• The newspaper can be in any language.
• The name change notice must be written in English.
• Applicant's old and new name
• Applicant's full address
• Applicant's father's/husband's name

CD Digital Format

• Digital CD created in MS Word format
• Old name in MS Word file
• New name in MS Word file
• Address in MS Word file

Government Fees Gazette

• Pay the fees through Bharat Kosh.
• The fees must be deposited with the Department of Urban Affairs.
• The fee category must be renamed.
• Print out the fee receipt.

Submission of Gazette File

Submit your Gazette name change application to the Publications Department and submit the following documents:

• Digital CD
• Affidavit
• Newspaper clipping
• Copy of fee receipt
• ID proof
• Witness form
• Gazette request application

Gazette Publication

• The Gazette is published 30 days after the file is submitted.
• Your new name will be published in Part IV of the Gazette.
• The Gazette will be valid throughout India.
• The Gazette can be downloaded from the Gazette website: (https://egazette.gov.in)
